Valir Hospice Care Llc in Oklahoma City is seeking an on-call Registered Nurse to support terminally ill patients in the OKC metro area. The role covers admissions to discharge with a focus on pain management and compassionate end-of-life care.

You will perform nursing assessments, update care plans biweekly, conduct 14-day supervisor visits, provide on-call coverage, and coordinate closely with the Patient Care Manager and medical staff to ensure high-quality, patient-centered care.

Qualifications

• Active RN license in good standing.
• Current CPR certification.
• 1–2 years of home health or hospice experience.

Responsibilities

• Oversees patient cases from admission to death/discharge.
• Completes nursing assessments for each patient assigned.
• Maintains patient medication profiles and updates charts.
• Updates the plan of care every two weeks.
• Performs 14-day RN supervisory visits on all assigned patients.
• Provides on-call hours as determined by Patient Care Manager.
• Communicates patient information to the Patient Care Manager and medical staff.
• Participates in in-services, staff meetings, and orientation.
• Assess, plan, implement, and evaluate patient care.
• Checks medication status and reorders as needed.
