Job summary

Chapters Health System is seeking an experienced RN for Nights and Weekends to perform after-hours field visits to patients, families, and community partners in the Marietta, GA area. The role focuses on establishing strong relationships with patients, families, and referral sources while delivering high-quality nursing care.

You will document care, collaborate with the interdisciplinary team, and ensure timely follow-up and safe discharges.

Qualifications

• Current RN license in the state of employment.
• Minimum of one year nursing experience; hospice or hospital experience preferred.
• Experience using EMR/EHR systems and basic computer skills.
• English communication required; bilingual in Spanish preferred.

Responsibilities

• Makes after-hours visits to patients in home, facility, or hospital; provides compassionate care.
• Assesses patient/family needs and initiates nursing interventions.
• Educates patients and families, documents care, and coordinates with IDG and physicians.
• Maintains care plans, ensures timely documentation, and supports referrals and admissions.
