Job summary

Three Oaks Hospice, Inc. is seeking a registered nurse to administer skilled nursing care for clients at their residence and coordinate care with the interdisciplinary team and families. The role involves assessing needs, educating patients on end-of-life care, and ensuring progress toward goals.

Qualified candidates will have an associate or bachelor’s degree in nursing, an active RN license, CPR certification, and strong communication and organizational skills. Hospice experience is preferred.

Qualifications

• Nursing degree and active RN license required.
• CPR certification and computer/MS Office skills preferred.

Responsibilities

• Perform initial and ongoing nursing assessments for patients at home.
• Coordinate care with the interdisciplinary team and family members.
• Educate patients and families on disease process and end-of-life care.
• Provide preventive and rehabilitative nursing procedures as needed.
• Document progress notes and care plans, updating as goals evolve.
• Schedule and participate in on-call coverage as assigned.
