Job summary

Three Oaks Hospice, Inc. is seeking a compassionate Registered Nurse to administer skilled nursing care for patients in their homes, coordinate care with the interdisciplinary team, and manage care plans.

The RN will support end-of-life care with dignity and clinical excellence. Responsibilities include assessments, implementing the plan of care, patient/family education, on-call coverage, documentation of progress notes, and collaboration with physicians.

Qualifications

• Graduate of an accredited Associate or bachelor's degree in Nursing.
• Current state license as an RN and current state Driver's license.
• Two years' experience as an RN in a clinical care setting; hospice or home health experience preferred.
• Current CPR certification.
• Proficient computer skills; Microsoft Office experience preferred.
• Strong interpersonal, communication, and organizational skills; high ethics and compliance.

Responsibilities

• Administer skilled nursing care for clients in their place of residence.
• Coordinate care with the interdisciplinary team and referrers; manage the plan of care.
• Conduct initial and ongoing assessments of patient needs and ADLs.
• Implement the individualized plan of care and revise as necessary.
• Educate patients and families about disease processes, end-of-life care, nutrition and self-care.
• Provide training to other staff as needed.
• Initiate preventive and rehabilitative nursing procedures.
• Prepare clinical and progress notes documenting progress toward goals.
• Coordinate services and prioritize needs with IDT; perform on-call duties.
