Three Oaks Hospice, Inc. is seeking an experienced Registered Nurse to provide skilled nursing care in patients' homes as part of a coordinated hospice team. You will work with the interdisciplinary team to manage care plans, assess needs, and educate families on end-of-life care.

The role requires a state RN license, CPR certification, and prior hospice or home health experience. Strong communication and organizational skills are essential for success in this patient-centered environment.

Qualifications

• Graduate of an accredited Associate or bachelors degree in Nursing.
• Current state license as an RN and current state driver's license.
• Two years experience as an RN in a clinical care setting; hospice/home health preferred.
• Current CPR certification.
• Proficient computer skills; Microsoft Office experience preferred.
• Strong interpersonal skills and ability to support patient and family.
• Demonstrate nursing practice skills and coping with family emotional stress.

Responsabilités

• Administer skilled nursing care for patients in their home.
• Coordinate care with the interdisciplinary team and referring agencies.
• Perform comprehensive assessments and develop care plans.
• Educate patients and families on disease process and end-of-life care.
• Provide training and support to staff as needed.
• Prepare progress notes and communicate patient changes to physicians.
• Coordinate services and prioritize patient/family needs with the IDT.
• Participate in on-call coverage as assigned.
