Job summary

Gentiva is seeking an experienced Hospice Clinical Liaison to ensure efficient intake and referrals, coordinate admissions, and strengthen relationships with hospitals, physicians, and community partners. The role emphasizes compassionate patient and family education, rigorous documentation, and cross-team collaboration to improve hospice access.

Ideal candidates have 3+ years of hospice experience, RN licensure in the employment state, and a BSN with leadership exposure.

Qualifications

• Minimum 3 years hospice experience
• Discharge planner/case manager experience preferred
• BSN degree preferred or clinical leadership experience
• RN license in the state of employment and CPR certification helpful

Responsibilities

• Receive and respond to referrals from hospitals, home health, physicians, case managers, SNFs, and other professionals
• Assess patient eligibility for hospice services by reviewing records and communicating with physicians and families
• Educate patients and families on hospice care and processes to provide clarity and reassurance
• Complete documentation and coordinate intake/admission to meet legal, clinical, and payer requirements
• Serve as liaison between referral sources and hospice care team for seamless communication
• Make daily visits to hospitals and facilities to maintain accessibility with discharge planners and physicians
• Build and maintain relationships with referral partners and identify outreach opportunities
