Job summary

Gentiva is seeking an experienced Hospice Clinical Liaison to coordinate intake, assess eligibility, and educate patients and families about hospice care. You will build strong relationships with hospitals, physicians, and referral sources while helping ensure smooth admissions and high-quality care.

The role requires a licensed RN with hospice experience, CPR certification, and a valid driver’s license. You will visit hospitals and care facilities regularly in a fast-paced environment.

Qualifications

• Licensed Nurse with 3–5 years of hospice experience required.
• BSN with 5+ years hospice experience preferred.
• Minimum of 3 years hospice experience required.
• Prior experience as discharge planner, case manager, or educator preferred.
• Current RN license to practice nursing in the state of employment.
• CPR certification.
• Valid driver’s license and automobile insurance.

Responsibilities

• Receive and respond to referrals from hospitals, home health, physicians, case managers, SNFs, and other professionals.
• Assess patient eligibility for hospice by reviewing medical records and communicating with physicians/families.
• Educate patients and families on hospice care and services.
• Complete documentation and coordinate intake/admission ensuring regulatory requirements.
• Serve as liaison between referral sources and hospice care team for seamless handoffs.
• Make daily visits to hospitals and facilities to stay accessible to discharge planners and physicians.
• Build relationships with referral partners and identify outreach opportunities.
