Gentiva is seeking a compassionate Hospice Clinical Liaison to ensure efficient intake and referrals, coordinating admissions and building strong relationships with referral sources. You will educate patients and families about hospice care, and visit hospitals and facilities to support discharge planning and access to services.

Ideal candidates are licensed RNs with 3+ years in hospice, CPR certified, and able to work across diverse settings while advocating for patient-centered care.

Qualifikationen

• Licensed RN with hospice experience
• BSN preferred for advanced practice
• CPR certification

Aufgaben

• Receive and respond to referrals from hospitals, home health, physicians, and other professionals
• Assess patient eligibility for hospice services through record review and clinical evaluation
• Educate patients and families on hospice care and processes
• Complete documentation and coordinate intake/admission while ensuring payer requirements
• Serve as liaison between referral sources and hospice care team for seamless handoffs
• Make daily visits to hospitals and facilities to collaborate with discharge planners
• Build and maintain relationships with referral partners and identify outreach opportunities
