Job summary

Gentiva seeks an experienced Hospice Clinical Liaison to streamline intake, assess eligibility, and educate patients and families about hospice services. You will coordinate admissions and foster strong connections with hospitals, physicians, case managers, and communities.

The role requires an RN license with 3–5 years of hospice experience; BSN is preferred. You will visit hospitals and facilities daily, ensuring compliant processes and compassionate care delivery.

Qualifications

• Licensed nurse with 3–5 years hospice experience.
• BSN with 5+ years hospice experience preferred.
• Minimum of 3 years hospice experience.
• Experience as discharge planner/case manager/educator preferred.

Responsibilities

• Receive and respond to referrals from hospitals, home health, physicians, case managers, SNFs, and others.
• Assess patient eligibility by reviewing records, conducting clinical evaluations, and communicating with physicians/families.
• Educate patients and families on hospice care, how it works, and why it matters with clarity and compassion.
• Complete necessary documentation and coordinate intake/admission per legal, clinical, and payer requirements.
• Serve as liaison between referral sources and the hospice care team to ensure seamless handoffs and rapid response.
• Make daily visits to hospitals and facilities, maintaining visibility with discharge planners and physicians.
• Build and maintain relationships with referral partners and identify new outreach opportunities.
