Job summary

Gentiva seeks an experienced Hospice Clinical Liaison to manage intake, referrals, and admissions with compassion. You will educate patients and families about hospice services, coordinate with discharge planners, and build strong referral partnerships across hospitals and care facilities.

The role requires an RN license with 3–5 years in hospice, plus strong communication and documentation skills. A BSN is preferred; CPR certification is required. Travel to facilities is expected.

Qualifications

• Licensed RN with hospice experience (3–5 years) required.
• BSN with hospice experience is preferred.
• Experience as discharge planner, case manager, or educator is a plus.

Responsibilities

• Receive and respond to referrals from hospitals, home health, physicians, case managers, and facilities.
• Assess patient eligibility for hospice services via medical records review and clinician discussions.
• Educate patients and families on hospice care, processes, and expectations.
• Complete intake and admission documentation to meet legal, clinical, and payer requirements.
• Serve as liaison between referral sources and the hospice care team for smooth handoffs.
• Make daily visits to hospitals and facilities to maintain visibility with discharge planners and physicians.
• Build and maintain relationships with referral partners and seek new outreach opportunities.
