Job summary

Gentiva Hospice is seeking an experienced Hospice Clinical Liaison to manage intake, assess referrals, educate families, and coordinate admissions with the care team. You will build professional relationships, support community awareness, and ensure smooth handoffs to the hospice staff.

Daily hospital and facility visits, collaboration with discharge planners and case managers, and strong communication skills are essential. RN licensed with CPR; BSN preferred; minimum hospice experience required.

Qualifications

• Licensed Nurse with three to five years of hospice experience required.
• BSN with five or more years of hospice experience preferred.
• Minimum of three years of hospice experience required.
• Prior experience as a discharge planner, case manager, or educator preferred.
• Current RN license to practice nursing in the state of employment.
• CPR certification.
• Valid driver’s license and automobile insurance.

Responsibilities

• Receive and respond to referrals from hospitals, home health, physicians, case managers, skilled nursing facilities, and other healthcare professionals.
• Assess patient eligibility for hospice services by reviewing medical records and communicating with physicians and families.
• Educate patients and families on hospice care, how it works, and why it matters.
• Complete necessary documentation and coordinate intake/admission to meet legal and payer requirements.
• Serve as liaison between referral sources and hospice care team for clear communication and rapid response.
• Make daily visits to hospitals and facilities to maintain visibility with discharge planners and physicians.
• Build and maintain relationships with referral partners and identify new outreach opportunities.
