Gentiva Hospice is seeking an experienced Hospice Clinical Liaison to ensure efficient intake and referrals, coordinate admissions, and educate patients and families about hospice care. You will build relationships with hospitals, physicians, and discharge planners to support seamless transitions and timely care.

The role requires an RN license with 3–5 years of hospice experience (BSN preferred), and travel to hospitals and facilities for daily visits while maintaining sensitivity and strong

Formación

• Current RN license to practice nursing in the state of employment.
• Three to five years of hospice experience required.
• BSN with five or more years of hospice experience preferred.
• Experience as discharge planner, case manager, or educator preferred.

Responsabilidades

• Receive and respond to referrals from hospitals, home health, physicians, case managers, SNFs, and other professionals
• Assess patient eligibility for hospice by reviewing records and communicating with physicians/families
• Educate patients and families on what hospice care includes, how it works, and why it matters
• Complete necessary documentation and coordinate intake/admission meeting legal, clinical, and payer requirements
• Serve as a liaison between referral sources and the hospice care team for seamless handoffs and rapid response
• Make daily visits to hospitals and facilities to maintain visibility with discharge planners and physicians
• Build and maintain relationships with referral partners and identify outreach opportunities
