Job summary

UnitedHealth Group is seeking an RN Case Manager for in-home hospice care. You will assess patients, develop and implement care plans, and coordinate with families, physicians, and the IDG to ensure compassionate end-of-life care. Travel within Atlantic and nearby counties is required, with on-call evenings and weekends as needed.

The role requires an active RN license, clean driving record, and Home-care experience. CPR certification is preferred or obtainable within 90 days.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle.
• Home-care experience is required.
• Preferred CPR certification or ability to obtain within 90 days of hire.

Responsibilities

• Assume full nursing responsibility for the delivery of the Plan of Care for hospice patients.
• Develop and implement treatment strategies promoting physical, psychosocial, and spiritual well-being.
• Perform initial nursing evaluation to determine eligibility for hospice services within 48 hours of referral.
• Identify needs and re-assess regularly, coordinate plan of care, and document patient progress.
• Collaborate with patient/family, attending physician, and IDG to provide daily care.
• Instruct and supervise patient/family in self-care techniques when appropriate.
• Maintain accurate clinical notes and adapt care to diverse patient circumstances.
