Job summary

UnitedHealth Group is seeking a Registered Nurse Case Manager for in-home hospice visits in New Jersey. You will deliver the Plan of Care, collaborate with an interdisciplinary team, and ensure compassionate, high-quality care to patients and families.

The role requires NJ RN licensure, a valid driver's license, Home-Care experience, and the ability to travel within a 1-hour service radius from Egg Harbor. On-call evenings and weekends are available.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.
• 1+ years of clinical experience.
• Current CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Makes the initial nursing evaluation within 48 hours of referral.
• Identifies patient/family needs and reassesses at least every 15 days.
• Initiates and coordinates the plan of care.
• Documents problems, goals, interventions, and responses.
• Collaborates with patient/family, attending physician, and IDG daily.
• Instructs and supervises patient/family in self-care when appropriate.
• Maintains accurate clinical notes.
