Job summary

UnitedHealth Group is seeking an RN Case Manager for in-home hospice visits across New Jersey. This PRN/part-time position requires current NJ RN licensure, a valid driver's license, and the ability to travel within a 1-hour radius from Egg Harbor, NJ to deliver comprehensive care.

The role includes performing initial evaluations within 48 hours of referral, coordinating plans of care, and collaborating with the interdisciplinary group to ensure high-quality patient care in diverse home settings.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.
• Preferred: CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Makes the initial nursing evaluation in determining eligibility for hospice services within 48 hours of referral.
• Identifies patient and family needs and reassesses at least every 15 days; initiates and coordinates the plan of care.
• Documents problems, goals, interventions and patient/family response to hospice care; collaborates with the IDG daily.
• Instructs and supervises patient/family in self-care techniques; maintains accurate clinical notes.
