Job summary

Brighton/Suncrest Hospice is seeking a Registered Nurse to provide direct patient care during home visits, SNF, and assisted living settings. You will join an interdisciplinary team to address psych-social needs and coordinate resources across the service area.

As an RN, you will contribute in Case Management, Admissions, or Resource roles, ensuring high quality care and adherence to hospice regulations. Travel is a part of the role with reliable transportation required.

Qualifications

• Current Registered Nurse (RN) licensure in good standing in state(s) of practice.
• Works well within a team to collaborate with nurses, social workers, chaplains, volunteers, and CNA leadership.
• Ability to work independently with phone, email, meetings, and various healthcare professionals while following hospice rules.

Responsibilities

• Provide care and services for a group of patients in homes, SNFs, and assisted living facilities.
• Contribute as part of the interdisciplinary team with psych-social assessments and community resources.
• Serve in Case Management, Admissions, or Resource roles within hospice operations.
