Job summary

Brighton/Suncrest Hospice seeks a Registered Nurse to provide compassionate care to patients in homes and facilities as part of an interdisciplinary team. You will contribute to psychosocial assessments, coordinate with case management, admissions, or resource roles, and travel across the service area as needed.

The role requires RN licensure in good standing, strong teamwork, and the ability to manage visits and documentation while delivering high-quality hospice care.

Qualifications

• Current RN licensure in good standing in the state of practice.
• Works well within a team with nurses, social workers, chaplains, volunteers, and CNA leadership.
• Ability to work one-on-one with patients while supported by a care team.
• Time management to handle visits, documentation, and meetings.
• Willing to travel across the service area with reliable transportation.

Responsibilities

• Provide care and services for a group of patients, visiting them where they reside.
• Collaborate with the interdisciplinary team on psych-social assessments and interventions.
• Work in Case Management, Admissions, or Resource role as assigned.
