Job summary
Top Echelon Software, LLC is seeking an RN Clinical Liaison for Hospice in the Greater Manchester, NH area. This full-time, Monday–Friday role focuses on hospital referrals and alliances rather than bedside nursing.
The position covers Bedford and Manchester-area facilities, requiring strong hospital relationship-building and a track record in healthcare sales or marketing. A valid driver's license and reliable transportation are essential.
Qualifications
- Active RN license required.
- Hospital bedside nursing experience required (med-surg preferred).
- Minimum 1 year of healthcare sales or marketing experience.
- Ability to build relationships with hospital clinical teams, discharge planners and physicians.
- Valid driver’s license and dependable transportation.
Responsibilities
- Develop and maintain relationships with hospital referral sources and healthcare professionals.
- Make regular calls to secure new referrals and meet established goals with physicians, facilities, case managers and social workers.
- Educate patients, families and clinical teams about hospice services and transitions of care.
- Support referrals, patient eligibility discussions and admission/consent processes as appropriate.
- Drive hospital referral activity and hospice census growth through service-focused outreach.
