Job summary
6 Degrees Search seeks a Hospice RN Clinical Liaison in Greater Manchester, NH. This full-time role focuses on building hospital referral relationships for Bedford and Manchester-area facilities, not bedside nursing.
Ideal candidates are clinically strong RNs with hospital bedside experience and 1+ year in healthcare sales/marketing. A valid driver’s license and reliable transportation are required.
Qualifications
- Active, unencumbered RN license required.
- Hospital bedside nursing background, including med-surg.
- At least 1 year in healthcare sales or marketing.
- Ability to build relationships with hospital clinical teams and case managers.
- Valid driver’s license, auto insurance and reliable transportation.
Responsibilities
- Develop and maintain relationships with hospital referral sources and healthcare professionals.
- Make regular calls to secure referrals, build census and meet goals with physicians and discharge planners.
- Educate patients, families and clinical teams about hospice services and transitions of care.
- Support referrals and admission/consent processes as appropriate.
- Drive hospital referral activity and census growth through outreach.
