Job summary

Ilocatum is seeking a Hospice RN Clinical Liaison in Greater Manchester, NH. This full-time role focuses on hospital referrals and builds relationships with hospital teams across Bedford and Manchester-area facilities, not bedside nursing.

The ideal candidate is an RN with hospital med-surg experience, oncology or hospice exposure preferred, and at least one year in healthcare sales or marketing. A valid driver's license and reliable transportation are required.

Qualifications

• Active, unencumbered RN license.
• Hospital bedside nursing background, including med-surg.
• Minimum 1 year of healthcare sales or marketing experience.
• Ability to build relationships with hospital clinical teams, discharge planners, case managers and physicians.
• Valid driver’s license, auto insurance and dependable transportation.

Responsibilities

• Develop and maintain relationships with hospital referral sources and healthcare professionals.
• Make regular, planned calls to secure new referrals and meet goals for referrals, census and collaborations with physicians, facilities, discharge planners and social workers.
• Educate patients, families and clinical teams about hospice services and transitions of care.
• Support referrals, patient eligibility discussions and admission/consent processes as appropriate.
• Drive hospital referral activity and hospice census growth through service-focused outreach.
