Essenti Talent is seeking a Hospice RN Clinical Liaison for Greater Manchester, NH. This hospital-focused role develops referral relationships with Bedford and Manchester-area facilities, not a traditional bedside nursing position.

The role requires an active RN license, hospital bedside experience, and at least 1 year of healthcare sales or marketing experience. A valid driver's license and dependable transportation are required for local travel.

Qualifications

• Active, unencumbered RN license.
• Hospital bedside nursing experience.
• Minimum 1 year of healthcare sales or marketing experience.
• Ability to build relationships with hospital clinical teams, discharge planners, case managers and physicians.
• Valid driver’s license, auto insurance and dependable transportation.

Responsibilities

• Develop and maintain relationships with hospital referral sources and healthcare professionals.
• Make regular, planned calls to secure new referrals, build census, and meet established goals on new and existing, referring physicians; facilities; case managers discharge planners and social workers in hospitals and skilled nursing facilities; and other health care providers.
• Educate patients, families and clinical teams about hospice services and appropriate transitions of care.
• Support referrals, patient eligibility discussions and the admission/consent process as appropriate.
• Drive hospital referral activity and hospice census growth through consistent, service-focused outreach.
