Job summary

Zaddy Solutions is seeking a Hospice RN Clinical Liaison to cover Greater Manchester, NH, Bedford and Manchester-area facilities. This full-time role runs Monday–Friday, 8:00 AM–4:30 PM, focused on building hospital referral relationships rather than bedside care.

The ideal candidate has an active RN license, hospital med-surg experience, and at least 1 year in healthcare sales/marketing, with strong relationship-building skills and a valid driver’s license.

Qualifications

• Active, unencumbered RN license required.
• Hospital bedside nursing background (med-surg) required.
• Minimum 1 year healthcare sales or marketing experience.
• Ability to build relationships with hospital teams, discharge planners, case managers and physicians.

Responsibilities

• Develop and maintain relationships with hospital referral sources and healthcare professionals.
• Make regular calls to secure new referrals and build census with referring physicians, facilities, and social workers in hospitals and skilled nursing facilities.
• Educate patients, families and clinical teams about hospice services and transitions of care.
• Support referrals, patient eligibility discussions and admission/consent processes as appropriate.
• Drive hospital referral activity and hospice census growth through service-focused outreach.
