Job summary

RRC Solutions Group seeks a Hospice RN Clinical Liaison in Greater Manchester, NH, to cover Bedford and Manchester-area facilities. This full-time role focuses on building hospital referrals and coordinating hospice care transitions, not bedside nursing.

The position requires strong RN credentials, hospital med-surg experience, and at least 1 year in healthcare sales or marketing, with a valid driver's license. Typically Monday–Friday, 8:00 AM–4:30 PM, on-site.

Qualifications

• Must hold active, unencumbered RN license.
• Hospital bedside nursing experience (med-surg).
• At least 1 year healthcare sales/marketing experience.
• Ability to build relationships with hospital teams, discharge planners, case managers and physicians.
• Valid driver’s license and reliable transportation.

Responsibilities

• Develop and maintain relationships with hospital referral sources and healthcare professionals.
• Makes regular calls to secure new referrals, build census, and meet goals with physicians, facilities, case managers discharge planners and social workers.
• Educate patients, families and clinical teams about hospice services and transitions of care.
• Support referrals, patient eligibility discussions and the admission/consent process.
• Drive hospital referral activity and hospice census growth through outreach.
