Job summary

Saint Cyr Partners is seeking an experienced Hospice RN Clinical Liaison to develop and maintain hospital referral relationships in Greater Manchester, NH. This full-time role covers Bedford and Manchester-area facilities and is not a bedside nursing position.

The candidate should have an active RN license, hospital bedside experience, and at least 1 year in healthcare sales or marketing, with strong relationship-building skills and reliable transportation. Monday–Friday schedule applies.

Qualifications

Active RN license required.
Hospital bedside nursing background (med-surg preferred).
Minimum 1 year of healthcare sales or marketing experience.
Ability to build relationships with hospital clinical teams, discharge planners, case managers and physicians.
Valid driver’s license, auto insurance and reliable transportation.

Responsibilities

Develop and maintain relationships with hospital referral sources and healthcare professionals.
Make regular calls to secure new referrals and meet goals with physicians, facilities, case managers and discharge planners.
Educate patients, families and clinical teams about hospice services and transitions of care.
Support referrals, patient eligibility discussions and the admission/consent process as appropriate.
Drive hospital referral activity and census growth through service-focused outreach.
