Job summary

The Sora Resource Group is seeking a Hospice RN Clinical Liaison to cover Bedford and Manchester-area hospitals in Greater Manchester, NH. This hospital-focused liaison role is full-time, not bedside nursing, and centers on developing referral relationships and guiding hospice admissions.

Qualifications include an active RN license with med-surg experience and at least 1 year in healthcare sales/marketing. A valid driver’s license and reliable transportation are required to support travel

Qualifications

Active RN license.
Hospital bedside nursing background (med-surg).
Minimum 1 year of healthcare sales/marketing experience.
Ability to build relationships with hospital clinical teams, discharge planners, case managers and physicians.
Valid driver’s license, auto insurance and reliable transportation.

Responsibilities

Develop and maintain relationships with hospital referral sources and healthcare professionals.
Make regular planned calls to secure new referrals, build census, and meet goals with physicians; facilities; case managers, discharge planners and social workers in hospitals and skilled nursing facilities; and other healthcare providers.
Educate patients, families and clinical teams about hospice services and appropriate transitions of care.
Support referrals, patient eligibility discussions and the admission/consent process as appropriate.
Drive hospital referral activity and hospice census growth through consistent, service-focused outreach.
