Hartford HealthCare at Home is seeking a Hospice Registered Nurse to provide skilled nursing care to patients in their homes within the Mansfield region of Connecticut. The role requires a CT license and at least one year of nursing experience, with preference for hospice, home health, or medical/surgical backgrounds.

The RN will perform comprehensive assessments, develop care plans, and coordinate with the interdisciplinary team to support end-of-life care and bereavement follow-up.

Qualifikationen

• Education: Associate's or diploma in nursing; CT license required.
• One year nursing experience or one year nursing experience in a specialty.
• Preferred: NLN approved BSN program; experience in hospice, medical/surgical, or home care.

• Provide skilled nursing care to hospice clients in their homes or community settings.
• Conduct comprehensive nursing assessment based on referrals and interviews.
• Develop and modify care plans in collaboration with clients and the care team.
