Job summary

Gentiva Hospice is seeking a dedicated RN Case Manager to lead in-home hospice care, performing initial and ongoing patient assessments and delivering skilled nursing interventions. You will develop and update plans of care, coordinate with physicians and the interdisciplinary team, and educate patients and families on disease progression and comfort measures.

As the primary contact, you’ll guide patients through the hospice journey, ensure timely documentation, and support quality improvement

Qualifications

• RN license in the state of employment.
• Minimum 1 year RN experience in hospice, home health, or related fields.
• Hospice or palliative care experience preferred.
• CPR certification and valid driver’s license.

Responsibilities

• Perform comprehensive patient assessments and reassessments with education for families.
• Deliver skilled nursing interventions and symptom management.
• Develop and update individualized care plans and visit frequency.
• Coordinate care with physicians and the interdisciplinary team.
• Educate patients and caregivers on disease, end-of-life care, and medications.
• Document timely clinical records and maintain continuity of care.
