Lower Cape Fear Lifecare is seeking a Hospice CNA to join our team in Lumberton, NC. The role involves providing direct patient care at patients' homes or facilities, charting in EMR when required, and coordinating with RN case managers to monitor patient status and family needs.

The ideal candidate holds a CNA certification, NC/SC licensure, and has a comfort level with travel and physically demanding tasks. A compassionate, patient-focused approach is essential to deliver quality hospice care.

Qualifications

• High school diploma or equivalent and completion of CNA course; 12 hours of in-service training supervised by RN on a rolling 12-month basis; onsite evaluation annually.
• Current certification with State Board of Nursing, or registration with Department of Facilities Service in NC/SC.
• Two years experience in hospice, home health or oncology preferred (no prior experience required).

Responsabilités

• Provide direct patient care such as bathing, peri-care, dressing, mouth care, toileting assistance, incontinence care, repositioning, and assistance with meals.
• Document bedside care and chart in facility EMR as needed.
• Coordinate with RN case manager to communicate patient status changes and family concerns.
• Assist with preparing and maintaining supplies in the vehicle; spend time driving between patients with mileage reimbursed.
• Instruct caregivers on personal care tasks while staying within scope of practice.
• Schedule visits with patients and caregivers, ensuring timely care across multiple sites.
