Job summary

Advocate Health is seeking an experienced RN Case Manager to coordinate patient and family care across hospice settings in Illinois. You will lead the interdisciplinary team, develop plans of care, and ensure documentation and compliance with state and federal guidelines.

The role emphasizes education for patients and families, timely care reviews, and collaboration with physicians and other IDG members to optimize quality of life at end of life. Strong nursing background required.

Qualifications

• RN license in the state where practice is required and BLS certification
• 1-2 years of experience in home health, hospice, medical/surgical, long-term care, emergency care/ICU or oncology or related experience
• Completion of an accredited nursing program and active licensure

Responsibilities

• Coordinate patient and family care in hospice settings to support end-of-life comfort.
• Educate patients and families on disease trajectories, hospice philosophies, and care plans.
• Review caseload to ensure appropriate utilization of services and level of care; make referrals as needed.
• Lead the interdisciplinary care group (IDG) to develop and implement each patients Plan of Care.
• Document in the electronic health record according to agency standards and policies.
• Assess patient response to treatments and adjust interventions; revise POC as needed.
• Supervise Home Health Aide and licensed practical nurse visits; instruct staff on regulations.
• Facilitate discharge planning and coordinate post-discharge resources as needed.
