JourneyCare Hospice is seeking a Hospice RN to join our dedicated team in Illinois. You will manage patient and family caseload across assessment, admission, planning, implementation and evaluation of nursing care.

Responsibilities include coordinating care with the interdisciplinary team, visiting patients regularly, documenting care plans, and supporting families through end-of-life care while ensuring quality and compassionate service. Competitive salary and benefits offered.

Qualifikationen

• Graduate from an accredited registered nursing program.
• Licensed as a registered nurse in the state of practice.
• Must possess current CPR and First Aid certifications.
• Knowledge of quality assessment and assurance procedures.
• Valid driver’s license and proof of insurance.

Aufgaben

• Assume primary responsibility for patient and family caseload through assessment, admission, planning, implementation, and evaluation.
• Gather data from patient and family factors that impact needs and coordinate with the interdisciplinary team.
• Initiate communication with attending physician and agencies to coordinate care.
• Provide regular visits to reassess needs and update the care plan.
• Perform clinical pronouncement of death and support the family.
• Maintain accurate patient records, charts, and care plans in a timely manner.
• Supervise and document delivery of care by Home Health Aide per guidelines.
