Job summary

Advocate Health in Illinois is seeking an RN to join the Hospice Southeast Suburbs team. The role involves coordinating care for terminally ill patients, educating families, and leading the interdisciplinary care group to ensure a consistent care plan and timely documentation.

On-call duties apply during specified holidays; regular hours are 40 per week, with a base pay range of $38.20–$57.30 per hour. Strong nursing experience and BLS/valid driver’s license are required.

Qualifications

• Typically requires 1-2 years of nursing experience in home health, hospice, medical/surgical, long term care, emergency care/ICU or oncology.

Responsibilities

• Identify and coordinate patient/family care to support terminally ill patients and families in hospice settings, with contacts at least every 15 days.
• Educate patients and families on disease processes, trajectory, hospice philosophy, and end-of-life care.
• Lead IDG care planning and ensure appropriate referrals, equipment, and services.
• Document in the Electronic Health Record and communicate changes to physicians and IDG.
• Supervise home health aides and licensed practical nurses per agency standards.
