Job summary

Chapters Health in Sebring, FL is seeking a Per Diem RN to assess patient needs using the nursing process, coordinate care with the IDG, and provide palliative and supportive care across hospice teams.

The role requires a current FL RN license, BLS, 1 year hospice or hospital experience, EMR/EHR experience, valid driver’s license, reliable transportation, and strong communication. Flexibility and emotional resilience are essential.

Qualifications

• Current license as RN in the state where the employee will be working.
• Active BLS for healthcare professionals from the American Heart Association or Red Cross.
• Minimum one (1) year nursing experience in hospice or hospital experience preferred.
• Previous experience working with an EMR/EHR (Electronic Medical/Health Record) system.
• Mobile Driver - Valid driver’s license and automobile insurance per Company policy.”
• Reliable transportation to meet visit schedule.
• Ability to use equipment with visual and auditory mechanisms.
• Ability to effectively communicate in English (verbal and written).
• Flexible schedule to meet the needs of Hospice House patients.
• Ability to manage the emotional stress of working with and caring for terminally ill patients and their families.
• Ability to physically access patient rooms, stairs, and various locations within the service area.
• Ability to perform essential functions and physical requirements (lifting, bending, etc.).
• Alzheimer’s Training certification for Florida affiliates may be required.

Responsibilities

• Provides direct care to patients and families as part of the IDG with appropriate nursing interventions.
• Coordinates the Plan of Care reflecting patient’s evolving needs.
• Educates patient and family about disease, palliative care, and safety practices.
• Reports changes in patient condition to IDG or health professionals.
• Participates with IDG to evaluate hospice referrals for level of care.
