Job summary

Chapters Health System is seeking an experienced RN to provide direct patient care in hospice settings across assigned facilities. You will coordinate care plans, educate patients and families, and work within an interdisciplinary team to support comfort and quality of life.

The role requires a Florida RN license, BLS, and at least one year of nursing experience (hospice/hospital preferred). A valid driver’s license and reliable transportation are necessary for visit-based care delivery.

Qualifications

• Current RN license in Florida.
• Active BLS for healthcare professionals.
• Minimum one year nursing experience; hospice or hospital preferred.
• Experience with EMR/EHR systems.
• Valid driver’s license and automobile insurance as per policy.
• Reliable transportation to meet visit schedule.
• Ability to lift and mobilize patients/equipment.

Responsibilities

• Provides and manages direct care to patients and families as part of IDG, with psychosocial, spiritual, cultural and physical components.
• Coordinates the Plan of Care reflecting the patient’s evolving needs.
• Educates patient, family, caregivers and other health professionals about disease, palliation and safety.
• Reports changes in patient condition to IDG or other health professionals.
• Attends patient deaths and prepares body for transport; oversees disposal of medications.
• Provides bereavement interventions with the family and caregiver.
• Presents concise oral and written reports to IDG; respects input from all disciplines.
• Communicates with physicians, staff, patients, families, and supervisors; maintains professional relationships.
• Supervises patient care by LPNs, Hospice Aides and contract nurses.
• During emergencies may be required to report to a designated location to ensure continuity of services.
