Job summary

Affinity Care of Sarasota is seeking a compassionate Registered Nurse to plan, organize and direct hospice care in home and community settings. The RN will leverage community resources to meet patient and family needs, provide education and coordinate care across disciplines.

The role emphasizes assessment, care planning, patient/family education, and collaboration with the interdisciplinary team. On-call duties and a valid Florida RN license are required.

Qualifications

• Graduate of an accredited school of nursing.
• Licensed RN in good standing to practice in Florida.
• Minimum 1 year of professional nursing experience within last 3 years; hospice/home health preferred.
• Valid driver’s license with insured vehicle.
• CPR certification required.
• Excellent observation, verbal and written communication skills.
• Ability to lift and transfer patients; prolonged walking/standing.

Responsibilities

• Perform initial and ongoing comprehensive patient and family assessments in home/hospice settings.
• Provide professional nursing care using the nursing process; implement plan of care.
• Develop and revise care plans with patient and family input.
• Coordinate with physicians; ensure timely orders and documentation.
• Educate patients and families on care, medications, and self-care strategies.
• Maintain safe home environment and participate in on-call duties.
• Supervise aides and manage caseload responsibilities.
• Participate in quality improvement activities.
