Providence at Home with Compassus is hiring a Hospice Registered Nurse for weekday day shifts in the Portland, Oregon area. The role involves working within an interdisciplinary team to provide routine and emergency assessments, care planning, and family education in the home setting.

The RN will manage symptom control, coordinate with the IDT, and ensure timely documentation. Requires an active RN license and CPR, with Hospice/home health experience preferred.

Qualifications

• Associate or Bachelor's degree in Nursing required.
• Minimum of one (1) year nursing practice or equivalent experience preferred.
• Hospice, oncology, home health and/or long-term care experience preferred.

Responsabilités

• Completes routine and emergency assessments on each patient.
• Provides clinical and psycho-social services as indicated by plan of care.
• Records observations, treatments, and other pertinent information.
• Communicates with IDT and physicians regarding symptom management and response to measures.
• Provides intruemental care and education to patient and family.
