Job summary

Qualifications

• Graduate of an accredited school of nursing.
• Registered Nurse with license in good standing to practice.
• Experience: minimum of 1 year within last 3 years OR BSN with minimum 2 years’ experience in public health, home care, or hospice preferred.
• Management experience not required; may supervise hospice aides.
• Offer contingent on references and background checks.
• Excellent observation, communication, and math skills; nursing skills per competency checklist.
• Physical ability to walk/stand for long periods and lift/move patients and equipment.
• Must be a licensed driver with insured vehicle.
• CPR certification required (hands-on verification).

Responsibilities

• Completes initial and ongoing comprehensive patient and family assessments to determine hospice needs.
• Provides professional nursing care using the nursing process.
• Writes and revises plan of care; regularly re-evaluates needs.
• Initiates and updates the plan of care as patient status changes.
• Develops a care plan based on nursing diagnosis and includes patient/family in planning.
• Delivers medications and treatments as prescribed; provides nursing care.
• Counsels patient and family on nursing and related needs; delivers health instructions.
• Assists with activities of daily living and promotes self-sufficiency and comfort care.
• Acts as Case Manager when assigned; coordinates patient care.
• Completes clinical notes and communicates with physicians; coordinates with community health providers.
• Maintains safe environment, participates in interdisciplinary group meetings.
• Participates in on-call duties and ensures equipment/needs are arranged.
• Supervises ancillary personnel and pursues ongoing professional development.
