Job summary

Providence at Home with Compassus is seeking a Hospice Registered Nurse to join a home-based care team in Medford, OR. The RN will perform routine and emergency assessments, educate families, and collaborate with the interdisciplinary team to support comfort and quality of life for patients and their families.

The role requires an RN license, CPR certification, and flexibility for on-call after-hours coverage.

Qualifications

• Associate or Bachelor's degree in Nursing is required.
• Minimum of one year nursing practice or equivalent experience preferred.
• Hospice, oncology, home health and/or long-term care experience preferred.

Responsibilities

• Completes routine and emergency assessments.
• Provides clinical and psycho-social services per plan of care.
• Records observations, treatments, and other pertinent information.
• Communicates with IDT, Medical Director, and attending Physician regarding symptom management.
• Educates patient/caregiver on care, disease process, dying process, symptom control, and wound care.
