Job summary

Providence at Home with Compassus is seeking a Hospice Registered Nurse (RN) to provide compassionate, person-centered care in patients0s homes. You will assess patients and families, deliver professional nursing care, and collaborate with the Interdisciplinary Group (IDG) to develop and direct individualized plans of care.

The role requires an active RN license, an associate or bachelors degree, and preferred hospice experience.

Qualifications

• Associate or Bachelorer degree from an accredited School of Nursing.
• Current, unencumbered RN license required.
• Preferred: 1 year of hospice or RN experience.

Responsibilities

• Complete patient and family assessments to determine Hospice needs.
• Provide comprehensive nursing care using the nursing process.
• Write and initiate Plan of Care with the IDG.
• Regularly reevaluate needs and revise the Plan of Care.
• Develop care plans with patient, family, caregiver, and IDG.
• May administer medications and treatments per Plan of Care.
• Participate in on-call duties and maintain productivity standards.
• Document clinical notes accurately and timely.
