Job summary

Elevation Hospice of Colorado is seeking a dedicated Registered Nurse to provide skilled in-home nursing care for patients across age groups. The RN will coordinate with the interdisciplinary team, patients, families, and referring agencies to ensure comprehensive, compassionate end-of-life care.

The role requires a state RN license, CPR certification, and a minimum of two years’ clinical RN experience (hospice/home health preferred).

Qualifications

• RN license in the appropriate state and a valid driver’s license.
• 2 years’ RN experience in a clinical setting; hospice or home health preferred.
• Current CPR certification.
• Graduate of an accredited nursing program.

Responsibilities

• Administer skilled nursing care for clients in their residence.
• Coordinate care with the interdisciplinary team and referring agencies.
• Perform comprehensive initial and ongoing assessments of patient needs and ADLs.
• Develop and implement the plan of care and revise as needed.
• Provide patient/family education about disease, care, and end-of-life options.
• Coordinate services and communication with IDT; on-call responsibilities as assigned.
• Maintain accurate clinical documentation and progress notes.
