Job summary

Elevation Hospice of Colorado is seeking a dedicated Registered Nurse to administer skilled nursing care at the patient’s residence. You will coordinate with the interdisciplinary team and family, ensuring high-quality end-of-life care in a compassionate, patient-centered approach.

The role requires an RN license, CPR certification, and hospice/home health experience. Strong communication, organization, and ethics are essential as you support patients and families through challenging times.

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state license as an RN and current state driver’s license.
• Two years’ experience as an RN in a clinical care setting; hospice/home health preferred.
• Current CPR certification.
• Proficient with computers; MS Office experience preferred.
• Strong interpersonal, communication and organizational skills.

Responsibilities

• Initial and ongoing assessments of patient needs and ADLs.
• Coordinate care with interdisciplinary team, patient/family, and referring agency.
• Educate patient/family on disease, end-of-life care, and self-care.
• Initiate preventive and rehabilitative nursing procedures.
• Prepare clinical and progress notes showing progress toward goals.
• Coordinate services and prioritize needs with the IDT; refer to other services as needed.
• Communicate changes in patient needs to physician and team.
• Provide on-call coverage as assigned.
• Other duties as assigned.
