Job summary

Qualifications

• Active Colorado RN license and CPR certification required.

Responsibilities

• Administer skilled nursing care in patients' homes across the territory.
• Coordinate care with the interdisciplinary team and family members.
• Document care plans, assessments, and progress notes.
• Educate patients and families on disease process, comfort, and self-care.
