Job summary

Reserve Care is seeking a compassionate hospice nurse to provide prescribed medical treatment and personal care to terminally ill patients across various settings, coordinating care under the physician's direction. You will work with a transdisciplinary team to implement the Plan of Care for each patient.

The role requires Ohio licensure, current CPR certification, and a valid driver's license with a good driving record; hospice experience is preferred and certification within two years may be

Qualifications

• Licensed in the State of Ohio and current CPR certification.
• Must have and maintain a valid driver's license with a good driving record.

Responsibilities

• Provide prescribed medical treatment and personal care in collaboration with the care team.
• Coordinate and implement the Plan of Care using the Nursing Process.
• Assess patient and family needs across physical, psychosocial, and spiritual domains.
• Monitor pain and symptoms; provide information and support to patient and family.
• Participate in care conferences, discharge planning, and on-call rotation.
