Job summary

Three Oaks Hospice, Inc. is seeking a dedicated Registered Nurse to provide skilled nursing care in patients’ homes within the Blythewood area and surrounding territory.

You will coordinate with an interdisciplinary team to deliver compassionate end-of-life care and support families in a range of settings. You will assess needs, implement plans, educate families, and participate in on-call coverage to ensure continuous, high-quality hospice services for diverse patients in the Northeast Columbia

Qualifications

• Graduate of an accredited Associate or Bachelor's degree in Nursing.
• Current state RN license and active driver's license.
• Two years of RN experience in a clinical care setting; hospice or home health experience preferred.
• Current CPR certification.
• Proficient computer skills; Microsoft Office experience preferred.
• Strong interpersonal, communication, and organizational skills; high ethics and compliance standards.

Responsibilities

• Initial and ongoing comprehensive assessments of the impact of terminal disease on the patient’s physical, functional, psychosocial, and environmental needs and ADLs.
• Implement the individualized Plan of Care (POC) and revise as necessary.
• Consult with and educate the patient/family regarding disease process, self-care techniques, end-of-life care, nutrition and dietary needs.
• Provide training to other staff as needed.
• Initiate appropriate preventive and rehabilitative nursing procedures.
• Prepare clinical and progress notes demonstrating progress toward goals.
• Coordinate all patient/family services and prioritize needs with the IDT members.
• Use case management approach and refer to other services as needed.
• Inform physician and other personnel of changes in patient needs and outcomes; evaluate response to care.
• Determine scope and frequency of services based on acuity and patient/family needs.
• Assess caregiver ability to meet patient needs at admission and throughout care.
• Evaluate own need for support and utilize available systems to meet it.
• Communicate information using current processes and technology available to the organization.
• Apply criteria for admission and re-certification to hospice care to establish appropriate levels of care.
• Perform on-call responsibilities and provide on-call service to patients/families as assigned.
