Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing
• Current state license as an RN and current state driver’s license
• Two years’ experience as an RN in a clinical care setting; hospice or home health preferred
• Current CPR certification
• Proficient with computer systems; Microsoft Office experience preferred
• Strong interpersonal and communication skills; ability to support patients and families

Responsabilités

• Administer skilled nursing care for clients in their residence and coordinate care with the IDT
• Implement the individualized plan of care and revise as needed
• Educate patients and families about disease process, end-of-life care, nutrition, and self-care
• Initiate preventive and rehabilitative nursing procedures
• Prepare clinical notes showing progress toward goals
• Coordinate patient/family services and prioritize needs with IDT and other providers
