Job summary

VITAS Healthcare is seeking an experienced Registered Nurse (RN) in Connecticut to coordinate care for hospice patients and families. You will perform comprehensive assessments and lead the interdisciplinary plan of care to address physical, psychosocial, emotional, and spiritual needs.

Role includes on-call coverage, on-site patient visits, and collaboration with the care team to ensure high-quality end-of-life care in the patient’s home or facility setting.

Qualifications

• Currently licensed to practice nursing in the state where the VITAS program is located.
• Minimum of two years of nursing experience in hospice, home health, or community health in the last five years.
• Equivalent experience or licensure may be considered.

Responsibilities

• Case management and supervision for Registered Nurse, RN.
• Follow VITAS policies and standards and practice within the scope of the RN license.
• Supervise nursing tasks delegated to Hospice Aides, Licensed Practical/Vocational Nurses and Volunteers.
• Complete initial and ongoing comprehensive assessments to palliate end-of-life symptoms.
• Identify and utilize referrals and resources (PT, OT, Home Health, Pharmacy, etc.).
• Document promptly at the time of events and at care locations.
• Educate patients, families, and caregivers on care plans, including pain management.
• Participate in patient care meetings and weekly team meetings.
• Provide shifts during non-business hours as needed and in program coverage area.
