Job summary

Qualifications

• Licensed to practice nursing in the state where the VITAS program is located.
• Minimum of two years of nursing experience in hospice, home health, or community health in the last five years.
• Equivalent experience or licensure may be considered.

Responsibilities

• Case management and supervision for RN care.
• Follow VITAS policies and standards and practice within the RN license scope.
• Supervise nursing tasks delegated to Hospice Aides, LPNs, and Volunteers according to the plan of care.
• Complete initial and ongoing comprehensive assessments to palliate end-of-life symptoms.
• Identify resources and referrals such as PT, OT, Home Health, and supplies.
• Document care events promptly at the time of service and at the care location.
• Provide education to patients, families, and caregivers, including pain management.
