Job summary

VITAS Healthcare in Waterbury, CT is seeking an experienced Registered Nurse (RN) to coordinate comprehensive hospice care for patients and families facing terminal illness. You will work within an interdisciplinary team to assess needs and implement the care plan, while maintaining documentation and quality standards.

The position requires an active RN license in CT and at least two years of hospice/home health experience.

Qualifications

• Current RN license in the state where VITAS operates.
• Minimum of two years of nursing experience in hospice, home health, or community health in the last five years.
• Equivalent experience or licensure may be considered.

Responsibilities

• Case Management and Supervision for Registered Nurse, RN
• Follows VITAS policies and standards and practices within the scope of the RN license and hospice eligibility requirements
• Reports any request to practice outside of scope of license to the manager/supervisor
• Performs supervision of nursing tasks delegated to Hospice Aides, Licensed Practical/Vocational Nurses and Volunteers
• Completes initial and ongoing comprehensive assessments for palliation of end-of-life symptoms
• Identifies/utilizes resources and referrals (PT, OT, Home Health, Dietary, Pharmacy, Equipment)
• Completes documentation at the time of the event and location of care
• Provides education and training to patients, families, and caregivers
• Participates in patient care meetings and weekly team meetings
• Provides shift reports through written/verbal communication
• Participates in bereavement support
• On-call rotation and shifts during non-business hours as needed
• Provides visits/shifts in the program coverage area
• Develops and follows the patient care plan with IDG
• Obtains physician orders as necessary
• Delivers primary nursing care and quality improvement activities
• Delegates tasks to Hospice Aides/Volunteers/LPNs appropriately
• Documents safety factors per CT health department guidelines
