Job summary

The VITAS Healthcare Corporation is seeking a Registered Nurse (RN) to join our hospice care team in Waterbury, CT. You will coordinate care, perform ongoing patient assessments, and implement the interdisciplinary plan of care to meet physical, psychosocial, and spiritual needs of patients and families.

Responsibilities include case management, supervision of nursing staff and aides, education for patients and families, and participating in team meetings and on-call shifts as needed.

Qualifications

• Current licensed to practice nursing in CT.
• Minimum of two years nursing experience in hospice, home health, or community health in the last five years.
• Equivalent experience or licensure may be considered.

Responsibilities

• Case management and supervision for RN.
• Perform assessments and implement the plan of care.
• Educate patients, families, and caregivers on care needs and pain management.
• Participate in weekly team meetings and on-call rotation as needed.
• Ensure documentation at time of events and care locations.
• Coordinate with IDG to develop patient-specific plans of care.
