Job summary

Compassus in Medford, Oregon is seeking a Hospice Registered Nurse to provide routine and emergency assessments, case management, and patient/family support in the home health setting. The role requires strong clinical and psycho-social skills and collaboration within an interdisciplinary team.

The candidate will educate patients and caregivers, maintain up-to-date documentation, and flex schedules to meet agency staffing needs, including after-hours on-call coverage.

Qualifications

• Associate or Bachelor’s degree in Nursing required.
• Active RN license in state(s) of employment required.
• Minimum of one year nursing practice preferred.
• Hospice, oncology, home health and/or long-term care experience preferred.

Responsibilities

• Completes routine and emergency assessments on each patient as indicated by departmental policy.
• Provides clinical and psycho-social services per plan of care through case management.
• Documents observations, treatments, and other pertinent information.
• Communicates with IDT, Medical Director, and attending Physician regarding symptom management and response to interventions.
• Provides physical and emotional interventive care to support patient and family during crisis.
• Coordinates hospice care with IDT to ensure continuity and quality of care.
• Updates the plan of care and communicates changes to the Hospice RNCM or DCS.
• Educates patient and caregiver about care, disease process, dying process, symptom control, and wound care.
