Job summary

Assured Hospice in Portland, Oregon, part of the Optum family, seeks a Registered Nurse Case Manager to deliver comprehensive hospice care and develop individualized care plans.

You will assess patient needs, coordinate with the care team, educate families, and document progress while ensuring compassionate, quality care across diverse settings. Current RN license, 1+ year experience, and driver's license required; CPR preferred.

Qualifications

• Current and unrestricted RN license in practice state.
• 1+ years of clinical nursing experience.
• Driver's license, valid vehicle insurance, and access to reliable transportation.
• CPR certification preferred or ability to obtain within 90 days.

Responsibilities

• Initial nursing evaluation within 48 hours of referral to determine eligibility.
• Identify physical, psychosocial, and environmental needs; reassess at least every 15 days.
• Initiate and coordinate the plan of care with the care team.
• Document goals, interventions, and patient/family responses to care.
• Collaborate with patient/family, physician, and IDG members daily.
• Instruct and supervise patient/family in self-care where appropriate.
• Maintain accurate clinical notes on patient condition.
• Provide care in diverse home settings regardless of background.
